Accidental Death and Dismemberment Insurance

Voluntary AD&D Insurance
When you enroll in the health plan, you are provided $10,000 of Accidental Death & Dismemberment (AD&D) Coverage provided by Unimerica.   You do not have to be enrolled in the health plan to purchase AD&D coverage.

Amounts of Insurance
Voluntary AD&D Insurance allows you flexibility in meeting your life insurance needs.   You may apply for any amount from $25,000 to $500,000 in increments of $25,000.   Any amounts in excess of $250,000 cannot exceed 10 times your annual earnings.   Amounts will be reduced beginning at age 70.

If you are insured for Voluntary AD&D Insurance, you may elect to insure your eligible dependents.   The amount of insurance for each dependent is determined as follows: Spouse only – 50% of the employee’s amount Spouse and Children – 40% of the employee’s amount for the spouse and 5% of the employees amount for each child Child(ren) only - 10% of the employees amount for each child, but not to exceed $25,000.
